Billing & insurance

For your convenience, we file almost every type of insurance, including Medicaid. Please check with your insurance plan for coverage. Billing questions can be directed to our billing representatives at 877-868-7594, weekdays from 8 a.m. to 5 p.m.

Depending on your insurance coverage you may owe out of pocket for the following:

• You are sick and we talk about your sickness (e.g., infections, joint problems, or abdominal pain) and plan how to make you better. You will be charged a copay or applicable deductible.
• We talk about your chronic illness (e.g., behavioral health, asthma, or ADHD) and/or prescribe medication or adjustments for your chronic illness. This is not covered as part of your annual visit.

We urge you to contact your insurance company to understand what is covered with your policy as every plan differs. If your insurance coverage does not include a particular screening, procedure, vaccination, or a problem oriented service performed at the time of a checkup the payment will be your responsibility.
